Keeping the Fun in Summer
One of the key elements of our pediatric services is integrating fun into healthcare for kids. So, we love the idea of your kids having a blast and getting active at summer camp! Whether it is hiking, swimming, canoeing or playing a contact sport, your child’s camp staff want to be able to protect kids while they are having fun.
So, in order for your child to participate, you may have to get an updated physical form filled out for your child. These forms help ensure that your child is kept healthy and safe whether they are enjoying challenging physical activities. Or, it may alert the camp of special needs your child may have, such as medications or a food allergy.
If your child is one of our patients, Sun Pediatrics can help you complete these forms with ease. Camp physical appointments are very similar to a well-check visit. Here are some of the things that might be required:
- Health and medical history
- Measurement of a child’s height and weight
- Physical exam of ears, nose, throat, lungs and abdomen.
- Up-to-date vaccine history
- Blood pressure, oxygen level, pulse and respiration
- Family medical history
- Overview of medications, prescriptions, and supplements
- Allergies or food intolerances
- Joint and muscle strength
- Flexibility
- Vision check
Summer camp forms and requirements do vary, depending on the type of camp that your child attends. But we will be happy to help you with any questions you have about the forms.
